Visit Information:
Initial office visits are 60 minutes with follow-up visits generally ranging from 15-45 minutes depending on condition and treatment.
For your first appointment, it is important that you do the following:
1. Bring completed new patient forms, which can be downloaded by clicking the appropriate of the links below. You only need one adult or child form, so choose the format that suites your needs. Everyone needs to view the privacy notice.

2. A complete list of medications including dosages. Also, bring the bottles or labels of any supplements, herbs, nutrients, etc. so that the actual ingredients can be viewed.
3. Copies of any lab work, x-ray reports, MRI reports, CT scan reports, EMG reports, etc. pertinent to your complaint(s) or generally completed in the last year. Bring a copy of the actual images if possible.
Most facilities will provide a copy of imaging on a CD/ DVD.
Medical Records Release: Use this form to request records from other your other doctors to be sent to Dr. Russo for your first appointment.
4. Insurance information.
-Please be prepared to pay your copay and be aware of the amount of your deductible
-If you have Medicaid, you need to have a referral on file from your primary care provider at the time of your first appointment.
-If you are covered by Worker’s Compensation or Personal Injury (auto insurance), you need to have your claim information including claim number, adjuster contact and referral/approval letter at the time of your first appointment
-Medicare does not cover our services
5. Dress Appropriately. Please bring clothing that allows for physical such as shorts and a sleeveless shirt that allows access to your arms and legs for physical examination. If you are coming for acupuncture and/ or manual therapy you will need to bring these each visit.
